ATTENTION MEMBERS:
Anyone who brings a new sponsor to Oak Creek Youth Football will recieve 10% of the paid sponsorship. You may bring as many new sponsors into the program by downloading the form (add your name to Referral line) and when the sponsor has been confirmed and is paid you will recieve a refund check. You may solicit as many sponsors as you would like and continue to add to your refund. Refund total many not exceed cost of registraiton per IRS guidelines.
***Existing no-relation sponsorship will be contacted by OCYF with no rebate reimbursement.
